How it works

  1. Personal details, goal, health history, injuries, medication, training experience and habits (sleep and stress).
  2. PAR-Q: 7 yes-or-no questions about the heart, chest pain, dizziness, joints and medication. Any “yes” means talking to a doctor before becoming more active.
  3. Click “Print / save PDF” to create the document with signature lines for your client and you. The data stays in your browser.

FAQ

What is the PAR-Q?

The Physical Activity Readiness Questionnaire is a screening questionnaire used worldwide before starting exercise. It doesn't diagnose anything: it flags who needs a medical check first.

Should I keep the signed form?

Yes, it's good practice. A signed form shows your client disclosed their health history and helps you plan safely. Update it when anything changes.

Is the data I enter saved on the site?

No. Everything stays in your browser and disappears when you close the page. Print or save as PDF before you leave.

In Trainer Fácil, the intake form is saved on the client's profile with their assessments and workouts.
