PERSONAL TRAINING SERVICES AGREEMENT TRAINER: ________________. CLIENT: ________________. The parties above enter into this personal training services agreement on the following terms. 1. SERVICES. The TRAINER will provide personal training to the CLIENT, 3 session(s) per week of 60 minutes each, at ________________. 2. FEES AND PAYMENT. The CLIENT will pay $280.00 per month, by day 5 of each month, via bank transfer. 3. LATE PAYMENT. If payment is more than 10 days late, sessions may be paused until it is received. 4. CANCELLATIONS AND NO-SHOWS. The CLIENT must give at least 12 hours' notice to cancel a session. Sessions missed without that notice count as completed. Sessions cancelled with enough notice can be made up within the same month, at an agreed time. 5. CANCELLATION BY THE TRAINER. Sessions cancelled by the TRAINER will always be made up at a date and time agreed by both parties. 6. HEALTH. The CLIENT confirms they have disclosed their health history on the intake form and will report any change. Where indicated, they will provide medical clearance to exercise. 7. TERM AND TERMINATION. This agreement runs for one (1) month and renews automatically for equal periods. Either party may end it with 30 days' notice, with no fee. 8. PRICE REVIEW. The fee may be reviewed once every 12 months, with at least 30 days' notice. 9. PHOTOS AND VIDEOS.
